2. Complete this disclaimer form, electronically sign, save & return to Emma via email.

When you work with Emma in any capacity, you make a vow to take full responsibility for your own experience - to meet yourself in sincerity, willingness, honesty and openness to the best of your ability. 

Please give minimum 24 hours notice of cancellation, otherwise you will be charged.
